The Pathways Project
Move Consulting was selected by the Active Partnerships National Organisation to work in collaboration with them on this Sport England funded project which seeks to develop pathways for physical activity in health and care systems.
The project aims to better connect the health and physical activity sectors in England, helping those living with, or at risk of, long term health conditions to benefit from being more active.

Our role is to work collaboratively with the Active Partnerships National Organisation to engage with stakeholders from across the health and physical activity sectors in England.
Using our Physical Activity for Health Framework as a guide, we will engage with organisations across the health and physical activity sectors to explore the information, guidance, tools and resources required to support those who are developing pathways locally.

There are four core stakeholders within the project:
The Active Partnerships National Organisation is the project commissioner. It is working in partnership with Move Consulting which is leading on the delivery of the project. The project is being overseen by APNO's Strategic Lead for Health, Annie Holden.
Sport England is funding the project. Through its Health and Wellbeing team, it is working collaboratively with the wider project team to ensure it is connected with wider strategic priorities within its Uniting the Movement strategy.
Coventry University is the project's research partner. It is led by Dr Nikita Rowley who is coordinating the research elements to the project and ensuring that we are exploring the available evidence base and developing evidence informed solutions.
Move Consulting is delivering the project. That's us! We are leading on the delivery of the project - engaging with stakeholders, facilitating the projects Communities of Practice and supporting them to reach into communities and networks across England to gather insights.
In addition to these core project stakeholders, we are striving to engage with as many physical activity and health sector organisations as we can, using this collective insight to co-design solutions.
The ultimate aim is to integrate physical activity into health and care systems.
Creating physical activity pathways to better connect the physical activity and health sectors has been recognised as key to realising this ambition, helping to unlock the value that is locked within community provision.
Through this project, we aim to highlight the key considerations to support a more consistent and coordinated approach to how organisations develop pathways and ensure they meet the needs of people locally.

Our health and care systems are under huge pressure. One in four of us have multiple long term health conditions.
And, by 2035, that’s expected to rise to two in three, over the age of 65. We believe this project could be a game-changer.
There are already pockets of good practice which shows that when health and physical activity come together, there are benefits for those with a long-term health condition. Plus, it can reduce service pressures and generate cost-savings for our health and care systems.
Thanks to Sport England funding, the project kicked off in February 2024 and will run until March 2025.
During this period, we will work collaboratively with the Active Partnerships National Organisation to engage with stakeholders from across the health and physical activity sectors so that, together, we can develop fit for purpose solutions to support the creation of pathways locally.
We need to gather insights and experiences from the health and physical sector so that we end up with a framework that is populated with the right information, guidance and resources that can be practically applied at a local level.
We need to;
Already, 39 organisations have joined one of five Communities of Practice. These Communities will not only help us identify examples of practice and key challenges but they will help us to co-develop possible solutions and ways of working by engaging with their local networks and sharing insights with us.

We cannot do this alone, so whether you are a community-based organisation, health partnership or national institute, we would love to hear from you.
National Partner Conversations
In addition to the communities of practice, the project team are engaging with recognised national partners from across the physical activity and health sectors. We want to ensure that we are aligned with developing policy. These organisations include NHS England, The National Academy for Social Prescribing, The Office for Health Improvement and Disparities and CIMSPA.

Pathways Perspectives (our sector-wide surveys)
Throughout the project, we will be running surveys that we call Pathways Perspectives. The surveys aim to gather further thoughts and insights from the physical activity and health sectors and we would love you to get involved.
